These individuals previously may have been arrested for public intoxication and/or taken to unnecessary psychiatric crisis programs. Clinicians and peer staff will engage clients to connect with ongoing substance and/or mental health services, ensure linkage and "warm hand-offs"-- - to other community-based services, and post-discharge follow-up.Address: 629 Main Street, Delano, CA 93215Hours of Operation: 24 hours a day, 7 days a weekAbout the Program: Telecare is proud to be partnering with Kern County Behavioral Health Recovery Services (BHRS) to operate the Delano Recovery Station in Delano, CA.The 6-client, short-stay, voluntary program will provide screening, access, and linkage to care for individuals with co-occurring mental illness and substance use needs. Services will be strength-based and person-centered and aim to promote recovery in a welcoming setting.The goal of the Delano Recovery Station is to provide a law enforcement diversion for persons who are acutely intoxicated and have a co-occurring mental illness where, instead of being arrested, they are presented with an opportunity for peer engagement, assessment, brief clinical interventions, and linkage with community-based services.POSITION SUMMARYThe Social Work Clinician III independently provides clinical and casework services to members served and natural supports. This involves person centered recovery planning and collaborating with other services and agencies. May act in the role of a clinical supervisor of unlicensed staff.Employment Status: ON CALLDays: VariesQUALIFICATIONSRequired:License or license eligible (LMFT, LCSW, LPCC, Licensed Psychologist, AMFT, ACSW, APCC)Master's Degree in Social Work, psychology, or equivalent from an accredited college or universityTwo (2) years of direct service experience in a behavioral health settingMust be at least 18 years of ageMust be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment and maintain current certification throughout employmentAll opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver license, a motor vehicle clearance, and proof of auto insurance is required at time of employment and must be maintained throughout employment. Additional regulatory, contractual, or local requirements may apply.Preferred:Previous experience with population to be served and/or work in a similar program settingTwo (2) years of post-licensure experience in order to provide clinical supervision to license eligible staffESSENTIAL FUNCTIONSDemonstrates the Telecare mission, purpose, values, and beliefs in everyday language and contact with internal and external stakeholdersConducts psychosocial assessments, treatment planning, group and individual clinical service, case coordination, and documentationIndependently completes clinical assessmentsEstablishes and maintains a therapeutic relationship with members servedEnsures provision of needed clinical services to assigned members servedCoordinates ongoing treatment within the program and with outside agenciesProvides individual and group therapyDocuments all treatment provided in a thorough and consistent mannerCollaborates with other staff in the discharge planning process and makes linkages to community services for members servedActively participates in multidisciplinary team meetings and recovery planning meetingsDemonstrates knowledge of de-escalation techniques and crisis communication and managementServes as a clinical consultant to recovery team membersMust evade members served in the event of assaultive behavior and pass assault crisis/crisis prevention trainingDuties and responsibilities may be added, deleted, and/or changed at the discretion of management.SKILLSAbility to complete social histories, risk assessments, mental health status exams, and plans of careAbility to work collaboratively with physicians, nurses, and rehabilitation therapistsAbility to form a therapeutic alliance with members served and familiesSufficient medical knowledge to assess the needs of members servedSkills in conducting group therapyAbility to advocate for the members served in other health delivery systemsPHYSICAL DEMANDSThe physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.The employee is occasionally required to sit, stand, walk, bend, push, pull, squat, kneel, and lift and carry items weighing 25 pounds or less as well as to frequently reach, twist, and do simple and power grasping.
