Job Description:
Compass Health, the largest provider of skilled nursing care in
SLO County, is looking for a LVN or RN with MDS experience to join
our team. The ideal candidate will be flexible to cover multiple
facilities in SLO County. Responsibilities of the MDS Nurse:
Oversee utilization of interdisciplinary rehab team process in
formulating MDS/RAI's. Observe direct nursing care, review
documentation, and report all discrepancies noted concerning
physician’s orders, diet change, charting errors, etc., to the
Nurse Supervisor Plan, schedule and revise the MDS, including the
implementation of RAPs, triggers, and care plans Complete the
entire MDS process in a timely and consistent manner. Ensure the
successful, timely electric transmission of the MDS package.
