About the Position Join Our Compassionate Team as a Hospice
Palliative Nurse Practitioner (NP)! Are you a dedicated Nurse
Practitioner with a passion for providing compassionate end-of-life
care? We are seeking a skilled Hospice Palliative NP to join our
home-based hospice team, making a meaningful difference in
patients' lives. This flexible, part-time role offers the
opportunity to work in a supportive environment dedicated to
high-quality palliative care, helping patients find comfort and
dignity during challenging times. What You’ll Do: Conduct
face-to-face evaluations for Medicare recertification in
collaboration with the Medical Director. Communicate effectively
with care managers regarding follow-up needs. Provide home visits
to patients in SNFs, care facilities, or private residences. Assist
with rounds in inpatient units as needed. Educate clinical team
members on care strategies to enhance patient comfort. Adapt your
schedule to meet the facility’s flexible timing, whether full days
or half days. Required Skills: Valid Nurse Practitioner license
with Palliative care experience. Pediatric experience is required.
Strong communication and documentation skills. Ability to work
independently and as part of a multidisciplinary team. Experience
with Medicare recertification processes. Nice to Have Skills:
Experience with home health or hospice settings. Prior inpatient
rounding experience. Knowledge of pediatric palliative care.
Ability to travel within Fresno and surrounding areas. Preferred
Education and Experience: Master’s degree in Nursing ( MSN or DNP).
At least 2 years of NP experience in hospice, palliative, or
related fields. Pediatric care experience highly preferred. Other
Requirements: Valid NP license in the state of California. Reliable
transportation for home visits. Willingness to travel and flexible
scheduling. CPR and basic life support certifications. Ability to
work 8-hour shifts, primarily days, with some availability for home
or facility visits. Compensation & Benefits: Hourly pay rate of
$78.39. Competitive pay packages with weekly pay via direct
