About the Position Join Our Team as a Part-Time Hospice Nurse
Practitioner – Make a Difference in Patients’ Lives Are you a
compassionate and experienced Nurse Practitioner passionate about
providing quality end-of-life care? We are seeking a dedicated
Hospice Nurse Practitioner to join our dynamic team, offering
flexible part-time opportunities to make a meaningful impact in
patients’ lives. Based out of Fresno, this role offers the unique
advantage of home-based hospice care, supporting patients in
various settings including homes, skilled nursing facilities, and
care communities. In this vital position, you will assist the
Medical Director with face-to-face recertification evaluations for
Medicare patients, communicate effectively with care managers for
follow-up needs, and provide clinical oversight or visits as
required. The role offers flexibility to tailor your schedule,
whether full or half days, to fit your availability, making it an

Skills: Current NP license in good standing Prior experience in
hospice, palliative care, or related fields Pediatric experience is
essential Strong communication and interpersonal skills Ability to
perform face-to-face Medicare recertification evaluations
Familiarity with care planning and patient education Nice to Have
Skills: Experience working with pediatric patients Certification in
hospice or palliative care Experience with inpatient rounding or
home visits Ability to collaborate with multidisciplinary teams
Preferred Education and Experience: Master’s Degree or higher in
Nursing (MSN or DNP) At least 2 years of clinical experience,
including hospice or palliative care settings Pediatric care
experience highly desirable Other Requirements: Valid driver’s
license and reliable transportation Willingness to travel within
specified areas Ability to adapt to flexible scheduling
Certification in CPR and other pertinent credentials This position
offers an hourly pay rate of $78.39, with benefits including
