Job Summary
Under the supervision of the Associate Medical Director or Medical Director, the Behavior Health Provider (PMHNP)will collaborate with PACE Physicians and Community-Based PACE Physicians to provide assessments, planning, and behavioral health. The Nurse Practitioner will routinely provide continuous behavioral health coverage to WelbeHealth participants in an interdisciplinary team environment while staying in accordance with all WelbeHealth standards. The Behavior Health Provider (PMHNP)collaborates effectively with colleagues and stakeholders to promote the our values, team culture, and mission.


Duties & Responsibilities:



  • Provide psychiatric consultation, psychiatric illness evaluation, counseling, and treatment for participants in collaboration with PACE Physicians and Community-Based PACE Physicians.


  • In collaboration with a physician, perform in-person or telehealth comprehensive psychiatric assessments on participants and collaborate with the IDT team to develop a comprehensive care plan for each participant.


  • Collaborate with physician(s) and IDT in discharge planning from acute or long-term care settings.


  • In consultation with a PACE Physician and/or Community-Based PACE Physician, refer participants to a psychiatrist.


  • In collaboration with the PACE Physician and/or Community-Based PACE Physician, manage care of participants in the nursing home, making regular visits as dictated by WelbeHealth standards and participant need.


  • Be a consultant to IDT communicating changes, collaborating care planning decisions and coordinate 24hr care delivery.


  • Collaborate with PACE and community-based PACE Physicians and IDT members to ensure effective care coordination.


  • Provides patient education regarding medications, risks, benefits, and reasonable outcome expectations.


  • Communicates with patients and engages in follow-up, as necessary.


  • May also provide primary medical care services, accordance with statues, regulations, protocols regulating the profession.


  • Maintain professional licensure and certifications.


  • Be legally authorized to practice in position location, required.


  • Only act within the scope of his or her authority to practice.


  • Complete all required documentation in a timely and accurate manner.


  • Protect privacy and maintain confidentiality of all procedures and info about team members, participants, and families.


  • Follow policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.


  • Communicate regularly with Supervisor and team regarding workload and priorities.


  • Timely completion of all mandated trainings and education.


  • Timely completion of all mandated occupational health screenings as needed.




    Job Qualifications


    • Education/Training: Master of Science in Nursing, with a Psychiatric/ Mental Health emphasis from an Accredited College or University; Certified as a Behavior Health Provider (PMHNP)


    • License/Certifications: Active Behavior Health Provider (PMHNP) State License without Restrictions; State CDS Certification; DEA License; NPI license; Valid State Driver's License with a clean DMV record; Copy of recent Vehicle Insurance required.


    • Experience: Minimum of two years' experience working with the frail or elderly.; Ability to build strong working relationships with other providers and care team.; Creative and enthusiastic commitment to the "triple aim" – dedication to improving experience, health, and PACE value; Current knowledge of chronic care/geriatric issues and best practices; Work well in a managed care environment with an interdisciplinary team (IDT); Experience and competency working with people from diverse backgrounds and cultures.


    • Skills: Excellent clinical, organizational, and communication skills with frail, elderly participants, their families, and IDT; Working knowledge of PACE regulations preferred; Ability to travel to other WelbeHealth locations; Excellent organizational and communication skills; Ability to work independently with minimal supervision; Demonstrated ability to prioritize in a fast-paced environment; Commitment to unlocking the full potential of our most vulnerable seniors.


    • Medical Clearance: Required that Team Members must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Additional screening may be required as determined by the Medical Director and Human Resources Director to protect participants and the community from communicable disease outbreaks


    • Regulatory Requirement: Must not be on any exclusion or debarment from participation in Federal Health Care Programs at any time and must remain in good standing with government regulators such as the OIG, (Office of the Inspector General), SAM, (System for Award Management), CMS (Centers for Medicare and Medicaid Services), etc.

    • Work Environment – Team Member must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants' care. The work setting is in an Adult Day Health Center, primary care clinic, other healthcare settings like hospitals, emergency departments and participant home environment with moderate noise levels and controlled temperatures.


    • Physical Requirements – Requires corrected vision and hearing to normal range. Must be able to communicate with all staff, vendors, and clients. Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment. Requires lifting/carrying up to 65 lbs. and pushing or pulling 150 lbs. using appropriate body mechanics and equipment, with reasonable accommodation if needed.


    • Visual, Hearing and Communication Requirements – Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by phone with participants, customers, vendors, and staff.


    • Pressure Factor – Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by Participants and/or their family members who may suffer from comorbidities such as dementia or mental health challenges.


    • Environmental Conditions – May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. May be subjected to unpleasant odors.
